Rapid Relief Massage Therapy Privacy Policy

Rapid Relief Massage Therapy is committed to protecting the privacy and confidentiality of personal health information (PHI). This Privacy Policy describes how personal information is collected, used, disclosed, protected, retained, and made available to patients in accordance with the Personal Health Information Protection Act, 2004 (PHIPA) and applicable professional requirements.

Who We Are

Rapid Relief Massage Therapy is operated by Tianna Rice, (RMT). Tianna Rice is responsible for the privacy of personal health information in her custody or control and serves as the contact person for privacy-related questions, access requests, corrections, and complaints.

Information We Collect and Why

We collect only the personal information that is reasonably necessary to provide safe and effective massage therapy services and to meet our legal and professional obligations. This may include identifying and contact information, health history, information related to your physical condition and function, treatment and assessment information, appointment and billing information, and information required for insurance or other payment purposes. Personal health information is collected, used, and disclosed primarily to:

  • Assess your health needs and provide massage therapy services.
  • Document and manage your care and communicate with you about your appointments and treatment.
  • Process payments and insurance claims where applicable.
  • Meet legal, regulatory, professional, and accounting requirements.
  • Carry out activities related to quality assurance, risk management, and the operation of the practice.

We will obtain consent for the collection, use, and disclosure of personal health information as required by PHIPA. PHI will only be collected, used, or disclosed for purposes that are permitted or required by law. Your PHI will not be used for marketing purposes without your express consent.

Disclosure of Personal Health Information

PHI may be disclosed to other health care providers, insurers, payment processors, service providers, or other parties when authorized or permitted by law and when reasonably necessary for the purpose. We may also disclose PHI when required or permitted by law, including for regulatory, legal, or public safety purposes. We limit the amount of PHI collected, used, or disclosed to what is reasonably necessary for the purpose.

Protecting Your Information

Reasonable physical, technical, and administrative safeguards are used to protect personal health information from loss, theft, unauthorized access, use, disclosure, copying, modification, or destruction.

Paper records, where applicable, are stored securely. Electronic records are protected using appropriate security measures, including password protection and encryption where appropriate. Access to personal health information is limited to what is reasonably necessary. Any service providers who require access to personal health information to provide services to Rapid Relief Massage Therapy are expected to maintain appropriate privacy and confidentiality safeguards.

Retention and Destruction

Patient health records are retained for at least 10 years after the patient's last visit, or, where the patient was under 18 at the time of their last visit, for 10 years after the day the patient turned or would have turned 18. When records are eligible for destruction, they are securely destroyed or disposed of in a manner that protects privacy and confidentiality.

Your Access and Correction Rights

You have the right, subject to limited exceptions under PHIPA, to request access to your PHI and to request correction of information that you believe is inaccurate or incomplete. Requests may be made by contacting Tianna Rice. We may require reasonable identification before providing access to PHI. Requests will generally be responded to within 30 days, as required by PHIPA, subject to permitted extensions. An access fee may apply where permitted by law. If a fee applies, an estimate will be provided before the fee is charged.

Privacy Concerns and Breaches

If you have a question or concern about how your personal health information has been handled, please contact Tianna Rice directly. Complaints will be reviewed and addressed promptly. If a privacy breach occurs, Rapid Relief Massage Therapy will take reasonable steps to contain and investigate the breach, address its effects, and prevent recurrence. Affected individuals will be notified as required by PHIPA. You may also make a complaint to the Information and Privacy Commissioner of Ontario (IPC) if you have concerns about the handling of your personal health information.
